Greater Tuberosity Fractures

A displaced isolated avulsion fracture of the greater tuberosity is indicative of a loss of function of the rotator cuff (Fig. 4.6a). In the two-part pattern, the bony fragment may retract under the acromion, acting as a block to abduction. To correct both the mechanical block and the loss of rotator cuff function, surgery is mandatory. We regard the displaced retracted greater tuberosity fracture as an absolute indication for surgery. Fig. 4.6a,b. Fracture of the greater tuberosity. a An avulsion-type fracture of the greater tuberosity. This is a true avulsion of the rotator cuff, in which the bone lodges under the acromion and acts as a mechanical block. Surgery is mandatory. b Fracture of the greater tuberosity has occurred at the time of anterior dislocation of the humeral head. In this situation, the tuberosity is in its correct position and does not retract. Therefore, reduction of the dislocation usually leads to an anatomical reduction of the tuberosity to...

History

The diagnosis of RFS is not always straightforward. Symptoms should be referable to and anatomically consistent with a history of prior radiation therapy. A complete history should include all pre-existing medical conditions including musculoskeletal and neurological conditions, such as rotator cuff tendonitis, cervical radiculopathy, neuropathy, and so on. The importance of pre-existing or developing musculoskeletal and neurological conditions should not be discounted because these can impact greatly the development of symptoms in the setting of radiation.
