After Spain lost the Spanish-American War (1898), Puerto Rico became a United States territory it later became a commonwealth (1952). Most Puerto Ricans only started migrating in increasing numbers to the United States in the 1950s as a result of rapid industrialization within the island and increasing unemployment. The absence of immigration restrictions, the cheap air travel between Puerto Rico and the United States, and the increasing numbers of family members in the United States also promoted this migration most Puerto Ricans went to New York City. Presentation of the Diagnosis and Treatment Plan to the Patient

If a psychiatric disorder is diagnosed after thorough workup of a patient with unexplained physical complaints, the first step is the presentation to the patient of the diagnosis and its implications for treatment. This is usually done by the primary care physician, sometimes together with the consulting psychiatrist. It may be of help to have family members present when this summing up takes place, to avoid any miscommunication. The physician should explain that the patients complaints do, indeed, have a cause stress, anxiety, depression, or whatever term is most accurate and acceptable to the patient. The physician may wish to reiterate that there is no doubt of the patient's suffering despite the absence of a medical or surgical disorder to explain it. Milestones are valuable because they provide a set of benchmarks for developmental progress. As I mentioned earlier, age by itself does not cause change to occur. Of the 672 homozygotes detected, 39.1 had significant hepatic iron loading and were therefore at risk for long-term complications. Cirrhosis was found in 5.6 of male and 1.9 of female asymptomatic homozygotes. Twenty-five patients in the study had repeat liver biopsies. In all cases in which cirrhosis was not present, removal of excess iron by treatment of disease resulted in improvement of the liver fibrosis score. This study shows that genetic-based screening is equally beneficial for the population as a whole and for family members of patients with hereditary hemochromatosis.

Dividual family styles and needs renders many family intervention programs ineffective, with families judged as uninterested when in fact it is the therapists that have failed. The third limitation is the failure of speech-language professionals to realize that families have other aspects of their lives that need attention, i.e., the activities of daily living, financial challenges, health concerns, other educational issues, and other family members. When you take a medical family history, you are inquiring about the very essence of an individual. You are asking not only about the individual's personal health, but also about their intimate relationships and the health of family members (with whom they may have little contact). Ideally, language materials are themati-cally related and relevant to the patient's daily needs and background. A large corpus of materials is recommended to vary the stimuli from session to session and to decrease practice effects. It is debatable whether the use of supplementary pictures or written sentences is appropriate (Helm-Estabrooks and Albert, 1991 Sparks, 2001). Frequent treatment, perhaps twice daily, is essential, but when unattainable, family members might be used to assist with the program (Sparks, 2001).

Let's consider a hypothetical gene that causes the presence of teeth at birth in one large family. When DNA samples from family members at risk of having the baby tooth trait were tested with marker M4, recombination events were seen to fall between the gene and the M4 allele only four percent of the time. If we have a map that shows the location of a set of genetic markers that includes marker M4, we can see that knowing there is a four percent recombination rate is not enough to let us place the gene on the map, since the gene could be located 4 map units to the left of M4 or 4 map units to the right of M4 (Figure 28.5).

My feeling is that a significant family history is one that the patient is concerned about, or one that the patient should be concerned about. A 22-year-old woman with an abnormal ultrasound finding at 16 weeks may have a strong family history of breast and ovarian cancer, but a desire for information about inherited cancer susceptibility testing is probably the farthest thing from her mind at that moment. Instead, delving further into this information at a future visit is warranted, and might be communicated to the woman in a follow-up letter.

Almost all kids have nightmares. They usually occur later in the night, during REM sleep, and they can be remembered at the time and in the morning. To prevent nightmares, discuss comforting things before bedtime and don't let your child watch violent or scary shows on television, especially right before sleeping. When nightmares do occur, remain calm and reassuring, encourage your child to talk about it, and stay in the room until she settles down and is ready to go back to sleep.
