In 1998, vascularized skeletal muscle allografting was done to cover a large defect in the scalp of a renal transplant patient who was on chronic immunosuppressive therapy (CsA and prednisone).20 After the second cadaveric renal transplant, squamous cell carcinoma developed in the scalp. The tumor was excised, and the defect was reconstructed with a cadaveric vascularized rectus femoris muscle flap. The patient received methylprednisolone, lg, and rabbit antithymocyte globulin, 1.5mg kg IV, before revascularization. He was returned immediately to his baseline immunosuppressive regimen of prednisone and CsA. A muscle biopsy 2 weeks after transplant revealed perivascular lymphocytic infiltration, and MMF (1000 mg PO b.i.d.) was administered, and the dose of CsA was increased. At 18 weeks, MMF was stopped, and the patient was kept on chronic CsA and prednisone. This patient is still under immunosuppression for kidney transplant with no reported complications.

Energy Metabolism

Another important enzyme associated with anaerobic metabolism is the oxygen-sensitive pyruvate-ferredoxin oxidoreductase (PFO) that converts pyruvate to acetyl CoA and C02 with the reduction of ferredoxin. Cryptosporidium PFO homologues were first found in the GSS entries from GenBank. Subsequent cloning and sequencing efforts revealed that the C. parvum PFO was part of a pyruvate-NADP oxidoreductase PNO (CpPNO) fused with a cytochrome P450 reductase (CPR) (Rotte et al., 2001). This type of gene fusion was coincidently found in another unrelated protist, Euglena gracilis. It is interesting that the E. gracilis PNO contains a mitochondrial transit sequence. However, CpPNO lacks such a mitochondria transit sequence, suggesting it could be cytosolic. Phylogenetic analysis has suggested a common ancestry of PFO in amitochondriate protists with Euglena mitochondrial PNO and apicomplexan CpPNO, which provides a new insight at the evolution of these unique proteins (Rotte et al., 2001)....

Pentoxifylline may decrease tissue injury of ischemia-reperfusion by inhibiting the production of platelet-activating factor during reperfusion.38 Pentoxifylline should improve microcirculatory dysfunction observed during reperfusion of ischemic tissues. Pentoxifylline causes increased deformability of RBCs and lowers blood viscosity.39 The optimal dosage appears to be 25 mg kg for protective effects in experimental studies in the canine gracilis muscle model. However, the dosage that produces maximal protective effects in humans is unknown.
