As a catabolic agent, muscle breakdown supplies carbon skeletons that the liver uses to synthesize glucose via gluconeogenesis. The combination of cortisol-induced insulin resistance at peripheral tissues such as muscle and adipose tissue, and increased hepatic glucose output produce abnormal glucose tolerance or frank diabetes. Muscle breakdown in the arms and legs leads to a myopathy manifested as reduced muscle size and weakness. Bone catabolism can be expressed as back pain, osteopenia, and pathologic fractures. Catabolism with thinning and structural weakness of the skin induces purple stria. Bruisability is a manifestation of vascular fragility from protein catabolism. Weakness of the abdominal muscles and increased intrabdominal fat produce a protuberant abdomen.
The abdomen should be examined with the patient lying comfortably on their back, with their arms by the side and neck and knees slightly flexed allowing the anterior abdominal wall muscles to relax. The patient should point to any area of discomfort or tenderness and this should be avoided initially during palpation.
The changes that occur in speech breathing with a switch from the upright position are numerous and reflect the different effects of gravity (see Hoit, 1995, for a comprehensive tutorial).
