Backpacks can be a helpful tool if they are used properly, since they can help children stay organized while toting their books and papers from home to school and back again. Compared to shoulder bags or purses, backpacks are better because the strongest muscles in the body, the back and the abdominal muscles, support the weight of

Patients who Have Dilated Cardiomyopathy Must Have a Trial of Bridge to Recovery

The development of cardiac assist devices has made much progress within the last 10 years. In the 1980s, mainly paracorporeal devices were available however, in the 1990s, the partly implantable and fully implantable pulsatile cardiac support systems emerged and played an increasing role (Fig. 1) 18,19 . Then, in 1998, the first rotary blood pump was implanted 20 . Rotary pumps are significantly smaller, free of noise, and have lower power consumption than the pulsatile devices (Fig. 2). Most of these devices are placed above the diaphragm and do not need a pocket between two abdominal muscle layers 21-23 . With these new devices, the assist device technology has reached a status whereby the surgical implantation procedure is easier than with the former systems, and infection one of the critical problems related to the size of the devices no longer plays a prominent role. The remaining problem with these pumps alludes to throm-boembolic events and potentially long-term effects caused...

Answers and Discussion

Q2. (Answer c) As stated above, strychnine interferes with normal activity of the inhibitory neurotransmitter, glycine. In doing so it produces spasms of the abdominal muscles, muscle twitching, and spastic paraplegia. The hyperexcitability induces convulsions, hyperthermia, and opisthotonus (an arched position of the body in which the victim lies on his back but only

Definition of the Disease

Cortisol in excess redistributes body fat to centripetal locations in the face, upper back, supraclavicular fossa, and abdomen. As a catabolic agent, muscle breakdown supplies carbon skeletons that the liver uses to synthesize glucose via gluconeogenesis. The combination of cortisol-induced insulin resistance at peripheral tissues such as muscle and adipose tissue, and increased hepatic glucose output produce abnormal glucose tolerance or frank diabetes. Muscle breakdown in the arms and legs leads to a myopathy manifested as reduced muscle size and weakness. Bone catabolism can be expressed as back pain, osteopenia, and pathologic fractures. Catabolism with thinning and structural weakness of the skin induces purple stria. Bruisability is a manifestation of vascular fragility from protein catabolism. Weakness of the abdominal muscles and increased intrabdominal fat produce a protuberant abdomen.

The abdomen should be examined with the patient lying comfortably on their back, with their arms by the side and neck and knees slightly flexed allowing the anterior abdominal wall muscles to relax. The patient should point to any area of discomfort or tenderness and this should be avoided initially during palpation.

Effects of Posture

The changes that occur in speech breathing with a switch from the upright position are numerous and reflect the different effects of gravity (see Hoit, 1995, for a comprehensive tutorial). In supine speech breathing involves approximately 20 less of VC. The change of body configuration from the upright position to the supine position means that rib cage volume decreases and abdominal volume increases. This modification changes the mechanics of the breathing muscles and requires a different motor control strategy for speech production. For example, in the supine position there is little or no muscular activity of the abdomen during speaking, whereas in the upright body position the abdominal muscles are quite active (Hixon, Mead, and Goldman, 1976 Hoit et al., 1988). In light of the mechanical and neural control issues discussed earlier, it seems unwarranted to position an individual supine to teach natural'' breathing for speech and voice. With regard to breathing at rest, it should...
